Postmenopausic bleeding caused by an uterine sarcoma. Case report.

Uterine sarcomas represent a group of aggressive neoplasms originated from several tissues, affecting the uterine stroma as well as the myometrium. According to the National Cancer Institute of the United States, it is estimated that these represent 1% of malignant gynecological tumors. Their most common clinical manifestations are vaginal bleeding, pain and the presence of a pelvic mass; however, they can be asymptomatic in 25% of the cases, depending on the histologic type. Here is presented the case of 74 years old patient, who states the current illness started on September, 2015 with post-menopausal bleeding in moderate amounts of two days of duration. Four months after, pelvic pain and the expulsion of a mass through the external genitals in three opportunities become associated to the previous symptoms, due to this she goes to Hospital Universitario de Caracas, where it is decided to admit her and do a transvaginal echography, where an endometrial pathology is diagnosed. Four months after that she undergoes an total extra-fascial abdominal hysterectomy with a bilateral salpingoophorectomy, and a pelvic and paraaortic lymphadenectomy; with a biopsy that reports a high- grade sarcoma of the endometrial stroma. Currently, the patient continues in annual control visits in the gynecology service, being free of disease. Taking into consideration the recurrence-free survival rate in patients older than fifty years of age is up to 20%, it highlights the importance that holds knowing the management and follow-up of this case.
